2022 School Physicals at UCHC
Union County Health Center will be providing school physicals for students on May 3rd and 5th from 8:30 AM to noon.
Students should arrive to the physical with the required forms filled out. Please prepare the following forms, included in the packet, in advance.
- Emergency Information Form
- Medical History Form
- Physical Examination Form
- Consent to Treat Form
