Medications are an important component for the care and treatment of patients at UCGH.

While not all patients will need medications, we have a pharmacy to provide necessary prescriptions.

To make sure that you are able to receive the medications you need when you need them, we ask that you carefully review the information on this page prior to visiting the UCGH Pharmacy.

If you are admitted to the hospital, your medication needs will be managed by your physician and nursing staff. In any case, if you are planning an overnight stay, be sure to bring your prescriptions with you from home just to be sure you have what you need.

Please note that the pharmacy is restricted to patient use only. UCGH is not authorized to fill outpatient prescriptions. Once released, medications must be obtained through your outside pharmacy.

Mail-in Prescriptions

  • Bring in your Refill Form with the Patient Portion completed
  • Provide the fax number, address, and phone numbers for the Mail-In Pharmacy you would like to use.
  • Please let you Doctor know at the time of your visit which pharmacy you will be using.

Prescription Refill

When phoning for refills, please have all information ready including:

  • Date of birth
  • Medication name
  • Dosage
  • Reason for taking the medication

PLEASE NOTE THE FOLLOWING:

Narcotics cannot be refilled without an appointment.

If you have more than three medications which need to be refilled, an appointment is required.

Please call at least 5 days prior to running out of your medication to ensure you can get an appointment in a timely manner.
